Crohn’s disease begins with crypt inflammation and abscesses, then progresses to microscopic localized aphthoid ulcers. These mucosal lesions can lead to deep longitudinal and transverse ulcers with intervening mucosal edema, giving the gut a characteristic cobblestoned look.
